- During the admission process, Sierra Care’s inter-disciplinary group of medical professionals work together to ensure a seamless transfer of care, uninterrupted therapy, and the establishment of a individual plan of treatment and rehabilitation to improve the quality of our patient’s lives.
- During in-patient stay at Sierra Care, in addition to providing the finest sub-acute care available, Sierra Care’s team of specialist nurses and other medical professionals will consult and collaborate with any additional medical specialists that optimum recovery and the patients individual needs may require.

Case Management
